A follow up will occur in apprx. 10 days for major violations #3, 12, & 20.

Verbal warning issued for evidence of smoking in kitchen. This facility has been written up 3 times for this violation. Smoking or tabacco use in food prep areas is prohibited!! Further violations of this nature may result in this facilitie''s license being suspended.
